Billing and Insurance
 
Marion Women’s Health Center has an excellent billing staff that can answer all your insurance billing questions. We will check your benefits and make you aware of any co-pay affiliated with an office or surgical procedure. In addition, we will check and verify your pregnancy benefits so you know exactly what to expect in terms of cost during this exciting time.

As a courtesy to our patients, we will pre-certify procedures and medications. For out-of-pocket expenses, we work with patients to develop a payment plan that works for each patient. 

Below are forms regarding your insurance coverage. Please check the appropriate box, fill out the corresponding form, print it and bring it with you to your appointment.

New Patient - Pregnancy Information
At your first visit, you will need to present your insurance card and a photo ID. If you do not have insurance, or we do not have a contract with your insurance company, a $550 deposit is required. If we have a contract with your insurance company, any co-payment designated on your insurance card will be collected at your first visit.  If you are under the age of 21, you must bring valid insurance cards for both of your parents, even if you have your own insurance.

After your first visit, our billing department will calculate a payment plan based upon the estimated amount that will be owed for your prenatal care and delivery.  You will receive a letter with the estimated amount that you will owe (if any) and payments will be set up on a monthly or per visit basis.

New Patient – Non-pregnancy/Gynecological Information

At your first visit, you will need to present your insurance card and a photo ID. Please make sure you fill out the forms provided in the Welcome Packet on the Patient Information page for Feminine Care. If you cannot complete these prior to your appointment, please check in with the receptionist 20 minutes before your scheduled appointment. This will allow time to fill out any additional paperwork that may be necessary and for your paperwork to be placed in your chart.

At each visit, you will need to present your insurance card and pay any applicable copay. If you do not have your insurance card or copay, you will be required to reschedule your visit.
